Upon hiring, complete and submit the following items:
- Act 34 Clearance – PA Criminal History Record
- Click on – Submit a New Record Check Individual Request – Reason for Request = Education. Print the report and submit.
- Act 114 Clearance – FBI Fingerprint Record
- Register with IdentoGO and enter service code 1KG6NX click go; schedule or manage appointment if you see an option to choose employment, select that option; enter your personal information and submit. Or you may call 1 844-321-2101 (Monday through Friday) to register for fingerprinting. Once you register, confirm your appointment with the fingerprinting site. After being fingerprinted, you will receive a receipt that contains your UEID number, which you will need to provide to us. The cost of the clearance will be payable at the fingerprint site by debit or credit. IdentoGo will not accept cash or checks as payment.
- Act 151 Clearance – PA Child Abuse History Record
- Print the report and submit.
- Act 126 Mandated Reported Training – Recognizing & Reporting Child Abuse
- Print the course completion certificate and submit.
- Applicable Certifications
- Arrest/Conviction Report & Certification Form
- School Personnel Health Record
- Sexual Misconduct/Abuse Disclosure Release (Act 168)
- Complete the top portion, section one, and submit. A separate form should be completed for each of the following:
- Current employer
- Former employers that are school entities
- Former employers where you held a position that involved direct contact with children

Culinary Arts Teacher
- Instructor will deliver food service instruction including ProStart training and operation of an in-house restaurant. Chef with management experience preferred. Demonstrated training ability and previous work with students and/or vocational instructional certification in food service is also preferred.
